We focus on understanding each young person’s story and meeting their needs in a way that feels stable, predictable and genuinely caring. The ACPP ensures they enter a home where trust is rebuilt, achievements are recognised, and their future becomes something they can shape with hope and support.
Introducing the Anchor Challenge Pathway Plan (ACPP)
The ACPP is Anchor’s structured, personalised framework designed to help every young person grow, heal and move towards independence with confidence. It provides clarity, stability and direction, ensuring each young person receives the right support at the right time throughout their care journey.
Why the ACPP Exists
Many young people arrive at Anchor having faced instability, trauma and disrupted relationships, often after multiple placements or time out of education and support systems. The ACPP was created to provide a clear, structured and compassionate pathway that gives every young person the stability and direction they may have missed. It ensures they receive consistent support from the moment they join, helping them rebuild trust, regain confidence and move towards a brighter future.
What the ACPP Involves
The Anchor Challenge Pathway Plan is a personalised development plan created within the first three months of a young person joining Anchor. Developed by a multidisciplinary team, it turns long-term goals into achievable steps using an annual plan that is then broken down into quarterly targets. This approach makes progress intentional, measurable and meaningful, ensuring that each young person receives support that reflects their needs, abilities and aspirations.
The Four Pillars That Guide Progress
The ACPP is built around four core pillars: Education, Health, Relationships and Development. Each pillar focuses on a key area of growth, from reconnecting with learning and building healthy routines to forming positive relationships and gaining the skills needed for independence. By assessing each young person against age-related benchmarks and identifying any barriers and gaps in experiences or learning, we tailor support across all four areas to help them grow confidently and prepare for adulthood.

Education
We support young people to reconnect with learning, build confidence and work towards realistic academic or career goals. This includes personalised plans, structured routines and close links with schools or tutors.
Health
We help young people look after their physical and mental wellbeing through healthy routines, emotional support and access to the right professionals. Our aim is to build positive habits and strong coping skills.
Relationships
We focus on helping young people develop safe, healthy relationships with staff, peers, family and the wider community. Through consistent care and guidance, they learn communication, trust and emotional resilience.
Development
We prepare young people for adulthood by teaching essential life skills, encouraging independence and building personal responsibility. This helps them feel confident and ready for life beyond care.

You will have your own bedroom and a comfortable home where you can relax, feel secure and settle into a routine. Staff are always around to support you, ensuring a stable and predictable environment from day one.

Support workers are available day and night — they listen, help you make plans, and guide you through anything difficult. A wider team including care staff, managers, therapists and education leads all work together to ensure you receive consistent, coordinated support from trusted adults.

Progress is reviewed monthly, quarterly and annually, with goals adjusted as you grow. By breaking targets into manageable steps, the ACPP helps you build confidence over time, develop resilience and take more responsibility for your own future.

If you feel worried, upset or unsure, there is always someone you can talk to. Staff are trained to help you understand your feelings and support you through difficult moments, so you never have to face anything alone.

What is the ACPP?
The Anchor Challenge Pathway Plan is a personalised development plan created for each young person. It sets out clear goals, milestones and steps that guide their growth in education, health, relationships and independence.
How soon is the ACPP created?
Each young person receives their ACPP within the first three months of joining Anchor. This early planning ensures we understand their needs quickly and begin supporting their progress from the start.
Who is involved in developing the ACPP?
A multidisciplinary team creates and maintains the plan, including care staff, home managers, the therapy manager and the education coordinator. Young people are encouraged to contribute to their own plan too.
How is progress monitored?
Progress is reviewed monthly within the home, quarterly by the Responsible Individual, and annually by the Senior Management Team. The ACPP is updated regularly to reflect achievements, new goals or any changes in need.
Will I have my own bedroom?
Yes. You will have your own room where you can relax, keep your things safe and make the space feel like your own.
Can I stay in touch with my family and friends?
In most cases, yes. Contact is based on what is agreed in your care plan, and staff will help you stay connected in a safe and supportive way.
What happens if I feel worried or upset?
There will always be someone you can talk to. Staff are trained to listen, help you make sense of how you feel and support you when things feel difficult.
Will I be able to do activities and hobbies I enjoy?
Absolutely. We encourage you to take part in things you like and try new activities. Staff will help you find enjoyable, safe things to do at home and in the community.
